Universal screening for thyrotropin (TSH) in pregnancy is cost-effective, preventing miscarriages and preterm births with modest population benefits. Extending screening to anti-thyroid peroxidase antibodies (anti-TPO) shows potential but requires stronger evidence and has a higher budget impact.
