Gastrosplenic Fistula in the Setting of Undiagnosed Lymphoma: A Case Report

Background:

• Gastrosplenic fistula (GSF) is a rare pathological connection between the spleen and stomach.

• This case report details a patient diagnosed with GSF and DLBCL, presenting with abdominal pain and weight loss.
• The patient achieved DLBCL remission but later developed respiratory failure due to a secondary abdominal-pleural fistula.

Findings:

• GSFs can lead to severe complications including massive upper gastrointestinal hemorrhage, infections, and other fistulas.
• Delayed diagnosis of GSFs is associated with increased morbidity and mortality.

Implications:

• Increased awareness of GSFs in the emergency setting is vital for prompt diagnosis and intervention.
• The complex nature of GSFs necessitates a multidisciplinary approach for effective management.
• Early detection and treatment are imperative to mitigate the high risks associated with GSFs.
